About the company
ASTRAMED-MS JSC (Medical Insurance Company) is a health insurer from Yekaterinburg that has worked in compulsory health insurance (OMS) since 1993, when the OMS system was created in Russia. It operates in the Sverdlovsk, Kurgan, Samara and Chelyabinsk regions, Perm Krai and the Republic of Bashkortostan.
As of 1 January 2026, 6,088,881 people held the company's OMS policies, including about 2.5 million in the Sverdlovsk region. Insured people are served by 233 policy issuing points, 362 insurance representatives and 461 medical experts; contracts are in place with 735 medical organisations. Astramed-MS has been a member of the All-Russian Insurance Association since 2017.
What a health insurance organisation and an OMS policy are
A health insurance organisation (SMO) operates in the compulsory health insurance system (OMS) under Federal Law No. 326-FZ. It issues policies, pays for the medical care of insured people out of OMS funds, checks the timeliness and quality of treatment and protects patients' rights, including through insurance representatives.
- The OMS policy is free — nobody should charge you for issuing it.
- The policy is valid throughout Russia: it entitles you to free medical care in any region.
- Today the policy exists primarily as an entry in the unified register of insured persons: the unique policy number, an extract or a digital policy on the Gosuslugi public services portal is enough; a paper form is issued on request.
How to choose or change your health insurance organisation
You can choose an SMO from the list of companies working in the OMS system in your region. The law allows you to change it once per calendar year — the application must be filed no later than 1 November — and more often if you move or if the SMO stops operating in the region. Apply at the office of the chosen company or via Gosuslugi; you will need a passport (a birth certificate for a child) and SNILS (individual insurance account number).

Voluntary health insurance
Besides OMS, the company holds a licence and rules for voluntary health insurance (DMS) (in force since 15.01.2025). According to the Bank of Russia, the company collected no voluntary insurance premiums in 2025: its core business is OMS, and OMS funds are not insurance premiums.
